If you have a version of CS2->CS4 with the Pen tool, or another program that allows Pentools and Vector masks, it's quite easy. It's still able to be made with another program just by leaving empty areas but the only requirement is the Ability to save as a .PNG Format.
Actually with the Pen Tool, you could make it alot more neater and attractive.
Start off by reading this Tut and understand how to use the Guides: [Link]
The Guides can be used to keep everything Symmetrical and straight.
Next, Make a set of Guides like this:

Now use the pen tool and outline the basic shape. Don't worry about any curves yet, just place the points where they need to go. After you have done that it's time to add the curves.
Hold Alt and hover the mouse over the point one of the points. You should see it change to a "<" like Symbol. This will allow you to add a curve.
Click and drag the point extension up to the corner.

You can see it in the extension made in the Top Left Corner. The Next image is without the Guides so that you can see it easier.

Now all you have to do is convert this to a Vector mask. Easy way is to then right click on the image and select "Create Vector Mask".
Tada, now you have your Border shape:

This Vector mask can then be applied to groups and other layers in order to hide anything that isn't within the selection. You can also use a new layer and the Edit->Stroke feature to create a simple border.
