it's a bit too empty
and the brush coming from the mouth doesnt go too well with it in my opinion

It's actually a c4d sharpened and set to color dodge.
But yeah, I get what you mean :)
Way too empty and on top of that you have a very weak background.
Place the text closer to the render.
Just use simple black or white borders.

You need to bring out the difference in your colors more. They're all a little too grayed out which makes it appear very flat. I also think she needs to be puking up more content, just to make things interesting =p And personally, I'd have her vomitting up the type too.
I also agree with Mega, except for the borders, I'd just drop the border in this case.
