To me it looks like you used topaz labs? More specifically portrait smooth.
Anyway cut that down a bit. It loses some detail from the piece.
I like the lighting in this a lot! I think you hit the nail on the head with this.
Depth is nice but I think the bg is slightly overblurred.
Like your colours as well
